By authorizing Willie The Bee Man, Inc., to perform services requested,you agree to pay the amount indicated
within this authorization invoice, as well as to acceptance of the terms and conditions set forth on the front and back
of this authorization invoice.
A. Our guarantee is limited in both time and scope. It is valid only in the event that the property owner has taken all
reasonable steps necessary to protect the property from any new swarm or re-infestation (including taking those steps identified
on the front of this authorization Invoice) and is limited solely to the removing of any new or remaining infestation or swarm
at the exact same location at no additional charge to you. Except as otherwise expressly provided for herein, no guarantee
or warranty is provided, and any implied guarantees or warranties are expressly waived by you.
B. You authorize Willie The Bee Man, Inc., to take any and all steps necessary to perform the services requested of us
and realize that, depending upon the circumstances presented, damage to your property may result from our efforts to remove
bees, wasps, hornets, and/or other stinging insects from your property or to perform the services requested of us. It is
further understood and agreed that Willie The Bee Man, Inc., does not repair damage that may result from our performing the
services necessary to remove bees, wasps, hornets, and/or other stinging insects, and that neither Willie The Bee Man, Inc.,
nor any employee, agent, or principal of Willie The Bee Man, Inc., is or shall be responsible for damage caused to your property
in connection with our performance of services or our removal of bees, wasps, hornets, and other stinging insects.
C. In the event of any dispute arising out of or otherwise related to your failure to pay all amounts due under the terms
of this Invoice, the prevailing party shall be entitled to recover any and all attorney’s fees and costs incurred,
including pre-suit, during suit, and on appeal.
D. The terms of this Invoice constitute an agreement between the parties, which shall be governed by the laws of the State
of Florida. Exclusive jurisdiction and venue relative to this Agreement shall exist within the Courts of Miami-Dade County,
Florida, and you hereby waive any and all claims of lack of jurisdiction over your person, improper venue, or related to forum
non conveniens.
E. The terms set forth within this authorization Invoice constitute the entire agreement between the parties and may
be amended only by subsequent agreement executed by the parties to be bound thereby.
F. Without limitation, in the event that payment is made by credit card and your credit card is declined, you agree to
pay any and all service fees charged by the credit card company or otherwise incurred by Willie The Bee Man, Inc., in addition
to any and all other amounts due and owing or all costs or attorney s fees associated with collection. Without limitation,
in the event that payment is made by check which is returned to us due to the unavailability of funds at the time due, you
agree to be responsible for any and all amounts provided for by applicable law, including, without limitation, bank charges
or other damages statutorily or contractually provided for, including costs or attorney s fees associated with collection.
G. You understand that bees, wasps, hornets, and/or other stinging insects present a danger to persons and animals alike,
and that the efforts undertaken by us to remove or exterminate such insects can aggravate their current state, thus increasing
the likelihood of those present being stung by such insects. Willie The Bee Man, Inc., is not responsible for, and you expressly
waive any potential liability or claims against Willie The Bee Man, Inc., caused by or associated with, (a) any damages or
personal injury that may be caused by (i) any person or animal being stung by any insect whatsoever or (ii) any actions that
may be taken by Willie The Bee Man, Inc., in an effort to remove bees, wasps, hornets, and/or other stinging insects as may
be requested of us, or (b) in the event that any future infestation or swarm appears at any location on or around the property
and any person or animal is stung as a result thereof.
I have been advised that the company's most common fees range between $195-$995 per beehive, and that, since the magnitude
of the hive cannot be ascertained from outside the structure, a firm quote cannot be given. I rely on the excellent corporate
reputation of WillieTheBeeMan, Inc., to charge me appropriately and fairly.
